How does it work?
We have a team of four full time support workers; one is a Punjabi/Urdu speaker.
Once we have received your referral, we will arrange a time to complete a needs assessment with you. This will be discussed with the team and if you have sufficient support needs you will be allocated a Support Worker as soon as possible.
The Support Worker will see you at an arranged venue on a regular basis. Support can last up to a maximum of 2 years depending upon your individual needs.
With your Support Worker you will identify risk through a risk assessment and how your needs can be met, by completing a support plan. You will then work together to achieve these goals.
